(Helena) – The Montana Department of Justice has uncovered the largest Medicaid fraud scheme in the history of its specialized control unit.
Attorney General Austin Knudsen announced that a judge ordered Kevin Patrick Maxwell, the former owner of Providence Mental Health Services in Bozeman, to pay the state nearly 2.4 million dollars.
An investigation revealed that between January 2022 and November 2024, the clinic used interns and unlicensed staff to provide mental health sessions, and then billed Medicaid for the services anyway. Investigators also found that the facility operated without a required mental health center license for part of that time.Maxwell closed the Bozeman clinic last year and moved to Texas before the civil complaint was filed under the Montana False Claims Act.
State officials remind residents that they can report suspected healthcare fraud, patient abuse, or exploitation by calling the Montana Medicaid Fraud hotline at 1-800-376-1115.
